Frequently Asked Questions

What can I expect on my first day?

You will work one-on-one with a licensed Physical Therapist who will take a detailed history, perform an assessment, review your personal goals for therapy and devise a personalized treatment plan.  Most of the time, treatment will begin on the first day so please bring or wear comfortable clothing.  Evaluations and treatment sessions will last about 60-75 minutes.

Do you take insurance?

Sage Physical Therapy and Wellness is a cash-based practice and is considered an out-of-network provider.  However, after each session, you can be provided with a "superbill" or itemized receipt, which details all of the information you need to submit a claim to your insurance company for reimbursement.  You may be able to receive reimbursement or credit towards your deductible or out of pocket amount.  We recommend reaching out to your insurance company prior to your first appointment with any questions about your out-of-network benefits.

Why do you not take insurance?

Providing care within a cash-based model allows Sage Physical Therapy and Wellness to provide customized care that is tailored specifically to each client without being limited by an insurance company.

Most insurance companies do not cover the entire cost of a physical therapy visit which leaves many patients responsible for copays and deductibles that can be up to $250 a visit until that deductible is met.  Sometimes, the patient may not be aware of this until they receive a bill several months after their treatment was completed.  Insurance companies also continue to lower reimbursement rates for physical therapy services making it necessary for physical therapy companies to make up the cost by increasing the volume of patients and double or triple booking their physical therapists.  This leads to less time with your licensed physical therapist, different therapists each session resulting in inconsistent care, and even portions of your care being overseen by non-licensed technicians.

By not taking insurance, we are able to provide personalized, one-on-one care with the same therapist every time, the ability to treat several body parts during the same care plan, transparency about the cost of every visit up front with no surprise bills, and more access to your physical therapist outside the treatment sessions.

Do I need a physician’s referral?

Michigan is a direct access state which means that a client can be seen for physical therapy over a period of 10 visits or 21 days before needing to be referred back their physician for continued care. However, some insurance companies require a script for all visits in order to reimburse.  Please check with your insurance company regarding their requirements and policy for out-of-network physical therapy reimbursement.

What forms of payment are accepted?

Payment is due at time of service.  We currently accept cash, check, credit card, HSA and FSA.  We advise that you review what is reimbursable with your specific insurance plans.

Do you take Medicare?

Medicare prohibits non-Medicare providers from taking cash for clients that are covered under Medicare.  We are currently not a Medicare provider and are not able to see Medicare patients at this time for physical therapy.

Where can you see me?

Sage Physical Therapy and Wellness is currently seeing patients out of the Southfield, Troy and Royal Oak offices of OMPT specialists.
